Employee Health Reporting Form
Butler University follows CDC guidelines for COVID-19 policies and procedures, and those are subject to change based on updated recommendations. The following FAQs are intended for all students who are experiencing symptoms related to COVID-19 or have recently tested positive:
Students are required to complete the Health Reporting Form if they have recently tested positive.
Yes, if you are a residential student with a meal plan. Read through the Isolation Instructions to locate the link for the food delivery survey.
Get tested and report on the form above if positive. If you would like to be seen at Health Services, please call (317) 940-9385 to schedule an appointment with a provider (not just a nurse).
Fill out your Health Reporting Form and report a positive test result.
- If you test positive for COVID-19, your isolation will last for five days following the day of your symptom onset or positive testing date if asymptomatic. You will be released from isolation after day five if you are asymptomatic or if your symptoms are resolving (without fever for 24 hours).
- We strongly encourage students who test positive for Covid-19 to isolate themselves at home or an off-campus location.
- Students who live in University housing (Irvington House, Residential College, Fairview House, Apartment Village, South Campus Apartments, Butler Terrace Apartments) who can safely go home for isolation are strongly encouraged to do so.
- If students remain on campus, they will be expected to remain in their assigned residence hall or apartment for the duration of their isolation and will be required to mask.
- Students living in fraternity and sorority chapter houses will be expected to comply with their respective chapter guidelines.
- If there are extraordinary circumstances or returning home is deemed a high-risk situation by Health Services, we have a very limited number of on-campus isolation spaces available.
Yes. Once you are placed in isolation by Health Services, the Dean of Students will provide your professors with the dates that you are unable to attend classes in person. They will not disclose any personal medical information.
- Butler University no longer has a campus-wide mask requirement.
- Students with a positive diagnosis are expected to wear a high-quality mask if they must be around others at home and in public.
- Continue masking days 6-10 after isolation release.
- Some students, faculty, and staff may opt to wear masks both in and out of the classroom. Masks may still be required by faculty and staff in classrooms, labs, or when meeting in private spaces. Individuals will be expected to comply with the requests of faculty or staff in these circumstances. The University will not tolerate disrespectful behavior toward any individual on campus regarding vaccination status, face masks, or any other reason.
